New Patient Registration
We are currently taking new patient registrations – do call past and have a look around if you are thinking of changing your GP or you have recently moved into the area. Our practice boundary covers most of the Richmond, South Kew and St Margarets areas. Please click on the link below for the map.
Please call into the Surgery and talk to any member of our Patient Liaison team to register. You will need to provide us with photo ID ( ie passport, driving licence ), proof of address and your NHS number, if you have previously registered with a NHS GP Practice.
We can only take new registrations before 6.00 pm on weekdays and up to 11.30 am on Saturdays.
PLEASE NOTE: YOU NEED TO BRING YOUR COMPLETED REGISTRATION FORM, QUESTIOANNAIRE, PHOTO ID AND PROOF OF ADDRESS TO THE SURGERY IN PERSON. WE DO NOT ACCEPT REGISTRATIONS VIA POST OR EMAIL.
Please note that patients are often removed from our list by the Primary Care Support Service, if contact details are not kept up to date. Do make sure that we are informed of any change of address or telephone number as soon as possible.
